One claim per engagement, and it applies when approvals came back inside 5 business days, the tracking stayed live (Always-Current Site Care or any plan keeps it live; hosting on its own doesn't), and at least 80% of the form and booking requests in those 90 days got a reply. We check that with you in your inbox and booking tool before any claim. Calls aren't part of it, because tracking sees the tap on your call button, not whether the call was picked up
